Provider Orders for Life Sustaining Treatment (POLST) 2017 – FACILITATOR Education Program @ Speare at Boulder Point Community Room

POLST-trained facilitators are often physicians, APRNs, social workers or nurses responsible for developing plans of care for residents of long-term care facilities, home health, or hospice. Without an emphasis on the importance of ACP discussions and qualified facilitators, completion of the POLST form runs the risk of repeating the failures of past standard AD efforts that have focused only on form completion.

POLST was implemented statewide in New Hampshire in 2013 and we are offering training to health providers who want to be certified as POLST facilitators for colleagues and staff in their organization and community.

The course focuses on communication skills— initiating POLST conversations, assisting in making informed end-of-life decisions, and learning techniques to create POLST plans. The successful implementation of a POLST program must be complemented by professionals who have developed the necessary communication skills to initiate advance care planning discussions that help individuals establish medical and non-medical goals of care.

  • Phil Lawson is a board certified hospice medical director and palliative care consultant with over 20 years experience in providing end of life care in New Hampshire.

Speare Shamrock Shuffle 5K 2017 Finish Line (15) horzWhether you are a serious runner getting ready for the upcoming race season, an avid walker logging steps on your pedometer, or just inspired to get up, get out, and get moving, Speare Memorial Hospital’s Shamrock Shuffle is the event for you.

This event was developed in response to an identified community health need to live a more active lifestyle. Speare’s Shamrock Shuffle 5K encourages participants of all ages and abilities. The event is not officially timed, as our focus is to encourage families and all community members to take steps toward a more active lifestyle.

Be sure to dress in green to celebrate the St.Patrick’s Day theme. Completing the course is sure to raise your heart rate and your spirits as we get ready to welcome spring. Proceeds from the event will support Speare’s community outreach and health education programs.

The first 150 registered walkers and runners are guaranteed their requested shirt size and the first 300 finishers will receive a Speare Shamrock Shuffle medal. The event will take place no matter rain, sleet, snow or shine.

Please Note: If you register in advance, you can pick up your packets from 3:00 – 5:00 p.m. in the Speare front lobby, on Friday, March 16th.

Whether you are participating or cheering from the sidelines, it’s sure to be a fun morning full of the luck of the Irish, in downtown Plymouth.
