We want to hear from YOU about what Your community needs!

Every three years, the Central New Hampshire Health Partnership conducts a Community Health Needs Survey for our region. This assessment helps inform our planning for health care services and other community health initiatives.

Please Click Here and take a few minutes to share your insights through this survey.

We understand that the current pandemic affecting our state and nation is a top concern for all of us. Along with you and many dedicated people across our region, we are working hard to keep our communities healthy and safe.  Hearing from you will help us identify the most important health needs not only now, but for the coming years.

We encourage you to make your voice count on behalf of the health of your family, friends, and neighbors.

The survey is completely anonymous and is intended for people age 18 or older.  You will not be asked for your name or contact information.

Please take 5-10 minutes to give us your thoughts and opinions. Survey results will help us shape our community health programs. Click Here to take the survey.
