Every three years, the Central New Hampshire Health Partnership conducts a Community Health Needs Survey for our region. This assessment helps inform our planning for health care services and other community health initiatives.

We understand that the current pandemic affecting our state and nation is a top concern for all of us. Along with you and many dedicated people across our region, we are working hard to keep our communities healthy and safe.  Hearing from you will help us identify the most important health needs not only now, but for the coming years.
