In addition to managing your own care, as a parent and/or health caregiver with authorized consent, you can have the ability to manage the records of those individuals for whom you provide care through proxy access.

To receive proxy access, complete the Proxy Authorization Form. You and the patient giving you proxy authorization will be required to come to the Medical Records Department in person at Speare Memorial Hospital. The completed Proxy Authorization Form, along with photo identification from both of you will be required to complete the appropriate authorization.
